Liability Release

As a participant, parent, or legal guardian of the child named above, I grant permission for their participation in the listed activities. Additionally, I authorize emergency medical treatment if necessary, without requiring prior approval. In exchange for this participation, I agree—on my behalf or on behalf of the child—to release GAMEON Tennis, its officials, employees, agents, and volunteers from any claims, liabilities, or damages arising from participation in these activities, whether known or unknown. I acknowledge that GAMEON Tennis is not responsible for lost or stolen items. Furthermore, I confirm that I am either the parent/legal guardian of the child or have obtained permission to enroll them in this activity. By signing below, I confirm that I have read, understood, and voluntarily accepted the terms of this waiver, fully aware of its implications.

Publicity Permit

Throughout the GAMEON Tennis Program classes and events, we are asked to take part in local publicity releases by way of newspaper articles, websites, Instagram, Facebook, television, and/or video. If you do, or do not, want your child’s picture or name to be used in such publicity releases, indicate below.
